The UPCC Membership and Board election ballot for the 2015-2016 period can be found here: 2015-16 membership form.

Please bring this form to the meeting on Thursday, June 11, 2015 or mail it to UPCC, 5511 17th Ave NE, Seattle, WA 98105. Ballots must arrive by June 11.
